Loss of Cervical Lordosis: What Does a Straight Neck Curve Mean?

Maybe you’ve had an X-ray taken and were told that you have a “loss of cervical lordosis” or a “straightening of the normal cervical curve.”

And your first thought was probably:

What does that actually mean?

This is something I talk about pretty often with patients, especially because spinal curves are an area I’m particularly interested in.

So let’s make it simple.

What Is Cervical Lordosis?

Your cervical spine is the portion of your spine that makes up your neck.

When viewed from the side, a typical cervical spine isn’t perfectly straight. It has a curve called a lordosis.

If you’ve ever seen me reviewing a lateral neck X-ray, this is one of the things you may see me looking at.

There are seven cervical vertebrae, and together their position contributes to the overall alignment and curve of the neck.

When that curve is reduced, we may describe it as a decreased or loss of cervical lordosis.

If the neck actually begins curving in the opposite direction, that’s different and may be described as a cervical kyphosis.

Does a Straight Neck Automatically Mean Something Is Wrong?

Not necessarily.

And this is an important distinction.

An X-ray finding by itself doesn’t tell me everything I need to know about a patient.

I don’t want someone to see an image of their neck, become scared because it doesn’t look “perfect,” and assume that one finding explains every symptom they’ve ever experienced.

Instead, I look at imaging as one piece of the puzzle.

Your symptoms, history, physical examination, range of motion, posture, neurological findings when relevant, and your goals all help provide context for what we see on an X-ray.

That’s why I evaluate the patient, not just the picture.

What Can Cause a Loss of Cervical Lordosis?

There isn’t one single explanation for every patient.

The appearance of the cervical curve can be influenced by a number of factors, including positioning during the X-ray. Changes may also be seen in association with injury, degenerative changes, or alterations in spinal alignment.

This is another reason context matters.

If I see a decreased cervical curve on an X-ray, I don’t automatically assume I know why it’s there or that it’s necessarily the source of someone’s pain.

Instead, I compare what I’m seeing with the rest of the patient’s findings.

Does Loss of Cervical Lordosis Cause Neck Pain?

This is where the answer isn’t as simple as social media sometimes makes it sound.

Research has examined relationships between cervical alignment, posture, neck pain, and function, but that does not mean every person with a straighter cervical curve will experience pain—or that every case of neck pain is caused by a loss of lordosis.

You can have an alignment change without significant symptoms.

You can also have significant neck pain without a dramatic change in the cervical curve.

So when someone asks me, “Is my neck curve causing my pain?” I don’t want to answer that question based on one X-ray measurement alone.

I want to look at the entire clinical picture.

Why Do I Pay Attention to the Cervical Curve?

Although I don’t believe every spinal variation needs to be “fixed,” spinal biomechanics are an important part of my postgraduate training and something I’m genuinely interested in.

I completed postgraduate certification training in Chiropractic BioPhysics® (CBP), which places an emphasis on posture, spinal alignment, biomechanics, and methods of structural rehabilitation.

Because of that training, I may evaluate spinal alignment differently depending on what a patient is hoping to accomplish.

For someone primarily seeking short-term symptom relief, our goals may look one way.

For someone who is specifically interested in their posture or spinal alignment, we may have a different conversation.

Neither patient automatically needs the same care plan.

Can You Change the Curve in Your Neck?

In certain appropriately selected patients, corrective approaches may be used with the goal of improving cervical alignment.

Depending on the patient’s findings, a corrective care plan may incorporate specific chiropractic adjustments, exercises, and cervical extension traction.

This type of care is different from simply adjusting someone’s neck because it hurts.

If we’re attempting to make a measurable structural change, I want to know what we’re starting with, what we’re trying to change, and whether we’re actually making progress.

That’s where reassessment becomes important.

What Is Cervical Traction?

If you’ve seen some of the equipment at Main Chiropractic and wondered what exactly we’re doing with it, this is where my CBP training comes into play.

Corrective cervical traction uses specific positioning and sustained forces based on the patient’s individual findings.

The goal isn’t simply to “stretch the neck.”

When appropriate, traction may be incorporated as part of a broader corrective program designed around a particular postural or spinal alignment goal.

That doesn’t mean everyone with neck pain—or everyone with a reduced cervical curve—needs traction.

It’s one tool, and I use it when the patient’s findings and goals make it appropriate.

Why X-Rays Can Matter in Corrective Care

If we’re talking specifically about spinal structure, I want objective information about what that structure actually looks like.

When clinically indicated, X-rays allow us to visualize aspects of spinal alignment that we can’t accurately determine just by looking at someone from across the room.

They can also give us a baseline.

If a patient chooses to pursue corrective care with the goal of changing spinal alignment, having objective measurements can help us evaluate whether we’re actually accomplishing what we set out to do.

Again, that doesn’t mean everyone needs X-rays.

It means that when imaging is clinically appropriate and the information will affect how I manage the patient, it can be a valuable part of the evaluation.

What Should You Do If You’ve Been Told You Have a Straight Neck?

First: don’t panic.

“Loss of cervical lordosis” is a description of what your cervical alignment looks like. It isn’t a complete diagnosis of everything happening in your neck.

The next step is understanding that finding in the context of you.

Are you experiencing neck pain?

Headaches?

Limited movement?

Did you recently have an injury?

Are you concerned about your posture?

Or did someone simply point the curve out on an X-ray even though you feel perfectly fine?

Those details matter.
